This service is distinct and doesn’t duplicate any other services the individual may already have.
Housing Transition
Housing transition services help individuals plan, find, and move into a community home by developing personalized plans, assisting with housing searches, and guiding lease negotiations. Additional support includes budgeting, organizing the move, and arranging necessary accommodations like adaptive housing or assistive technology. These services ensure a smooth and stable housing transition.
Housing Sustaining
Housing sustaining services ensure long-term housing stability by regularly updating support plans, preventing behaviors that may risk housing, and educating tenants on their rights. Services include building relationships with landlords, eviction prevention, income support, and assistance with recertification. We also help with household management, cultural needs, and arranging adaptive housing or assistive technology for safe living.
